No you will not have to cancel your previous health insurance policy yourself. If you decide to switch insurance providers before the 1st of January, your current insurance policy will be automatically cancelled. The new insurance policy will be activated on the 1st of January. You are entitled to a cooling-off period of 14 days in which you may cancel the new insurance policy.
